My friends progress got deleted on my server!?

So my friend has been having some trouble on my server.  He has been getting black walls that restrict acess to other parts of the map.  I thought it was an issue with my ports, but after checking it I realized there was nothing wrong.  He has to quit and rejoin in order to fix this issue.

 

Now, the last time he did this, he was asked to make a new character upon entering my server.  All his progress had suddenly...vanish!  What happened to it?  Is there any way to get his character back?  Finally, is it a problem with my server, or is it a problem on his end?

 

This is the second time this has happened.  Any help or insight would be extreamly helpful!

black barders happen, his save is on his pc not ther server, and it is possible he closed the client by hitting the X instead of the right way and his char stayed on the server ad died.

Turns out he has NEVER quit the game the right way.  He actualy didn't know about it *facepalm*.  But his character never remained on the server and died.  I know that because I was always next to him when he had to restart due to the black borders.  I saw his character dissapear, not suddenly die.  it worked fine a couple of times.  Could it be possible that his files got corrupted because he didn't exit out the right way?
