Need help on creating my trait mod

I have created this mod that adds a tourette syndrome trait to the game it currently works in singleplayer but im trying to get it to work with in multiplayer and I cant figure out what im doing wrong. Its my first mod for project zomboid or any game. Any help would be appreciated. 

 

The mod shows up and I can select it when creating the character but it doesnt do anything in-game
