You need to disable anticheat type 21 in the server options. Just had this happen on my server and turning it to false worked 

Edit: Last night at around 11 PST everyone from my server was booted due to type 22 seemingly out of nowhere, disabling type 22 fixed it.

Sadly the hotfix doesn't fix, server crashed.

Attaching the log file and will try to get the big file.

You should have gotten the link to the zipped up server, we also tried with a completely fresh map, but it was the same.
